Brief Description of Services Offered:

Residential recovery treatment program (Voluntary- Level 4) (length of stay approx. six mos) Transitional living structure- mothers work within the community, participate in weekly substance abuse/relapse prevention group counseling & individual counseling. The goal is to get back on their feet, strengthen their recovery in a structured environment, and ultimately live independently with their children.

Criteria:

• For mothers in substance abuse/alcohol recovery (at least 30 days clean).
• Mothers must have children under the age of 10 and/or be pregnant.
• Can have a DCF case plan with the goal of reunification.

Exclusion Criteria:

• No Methadone or other controlled medication.
• Subutex treatment is okay on a case-by-case basis (must be within the COC MAT program).
• Any mental health issues must be stably managed and not of primary concern.

Is Insurance required? No

Referral Process:

• Complete application packet located HERE.
• Send completed application packet to Valerie Brooks at vbrooks@circlesofcare.org.
• A face-to-face screening interview will be conducted to confirm eligibility and place on the waiting list (if no immediate availability)
