Brief Description of Services Offered:

The MAT Program is dedicated to helping those in our community who desire to come out of the addicted lifestyle, into a more rewarding and stable way of life. MAT is an intensive outpatient program with wraparound services. Our services include Individual and Group therapy, prescription medication along with medical visits, Case Management services, and Peer Support services. Our team is passionate about assisting individuals to reach their personalized Recovery goals that will lead to a more healthy and productive lifestyle.

Criteria:

• 18 or older
• A desire to reduce or eliminate the use of Opiates or ETOH.
• Willingness to fully participate in all services offered by the MAT Program (Medication, Therapy, Case Management, and Peer)

Exclusion Criteria:

• Pain Management Services
• Pregnancy

Is Insurance required?: No. All services for uninsured individuals will be covered by Circles of Care.
If an individual has insurance, we are required to bill the insurance but an individual will not be turned away due to financial concerns or an unpaid bill.
